Position Overview

The Division of Mental Health is seeking to hire a Social Worker III for the Elgin Mental Health Center located in Elgin, Illinois to provide professional social work services to patients served at Elgin Mental Health Center (EMHC). Performs professional social work services to patients served. Completes initial, addendum and annual social assessments. Serves as a member of the interdisciplinary treatment team. Serves as a team leader over an assigned team in the Forensic Unit. Coordinates services as well as provides referral and linkage to outside resources. Attends patient's treatment plan meetings, discharge planning meetings and required training. Performs social work functions in accord with the Department of Human Services (DHS) and EMHC policy and procedures.

Essential Functions

  • Provides professional social work services to patients served at the Elgin Mental Health Center (EMHC).
  • Serves as a team leader of the social service team and member of the interdisciplinary treatment team.
  • Serves as the primary team liaison through personal meetings and correspondence with other team members, guardians, courts, family, and community agencies.
  • Coordinates services as well as provides information for annual evaluations.
  • Provides direct patient counseling as part of patient programs and/or legal rights issues.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.

Minimum Qualifications

  • Requires a master's Degree in Social Work from a recognized college or university supplemented by two (2) years of professional social work experience in the evaluation and treatment/habilitation of intellectually and developmentally disabled, or other disabled persons.
  • Requires Illinois licensure as a Licensed Clinical Social Worker by the Department of Professional Regulation.

Conditions of Employment

  • Requires the ability to walk and stand for a significant period of time, including stooping, bending, lifting or exerting up to 20 pounds of force occasionally.
  • Requires the ability to physically restrain patients as necessary to prevent injury to patient or others.
  • Requires the ability to utilize office equipment, including personal computers.
  • Requires ability to pass the IDHS background check.
  • Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
