Position Overview

The Division of Developmental Disabilities is seeking to hire a Social Worker IV for the Choate Mental Health and Developmental Center located in Anna, Illinois to Serve as a Program Coordinator for Interdisciplinary Teams on the Unit. Serves as a qualified examiner and signs certificates of admission. Develops, integrates, coordinates and monitors Individual Habilitation Plans (IHP). Provides evaluation, testimony and coordination with courts regarding individuals subject to judicial admission. Coordinates with individuals served and/or guardians regarding administrative admission status. Performs highly responsible casework including program compliance assessments, pre-admission screenings, admissions, social evaluations and discharge planning.  Serves as contact individual for community agencies, public and private, providing consultations that enhance the knowledge of individuals who are developmentally disabled and the resources available to them.  Serves as a lead worker to lower-level staff.

Essential Functions

  • Serves as a Program Coordinator for Interdisciplinary Teams on the Unit.
  • Serves as QIDP (Qualified Intellectual Disabilities Professional) social work specialist for the Interdisciplinary teams. 
  • Serves as a qualified examiner and signs certificates of admission.
  • Serves as designated lead worker to lower-level staff.
  • Conducts formal and informal individual, group and family therapy, makes notes regarding results and provides information to the treatment team in maintaining and updating treatment planning.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.

Minimum Qualifications

  • Requires a master’s degree in social work from a recognized school of social work supplemented by three (3) years of supervised post master’s clinical social work practice and possession of a clinical social worker’s license from the Department of Professional Regulation.

Conditions of Employment

  • Requires the ability to meet criteria for a Qualified Intellectual Disabilities (QIDP) within his/her probationary period.
  • Requires the ability to utilize office equipment including personal computers.
  • Requires the ability to work after business hours, weekends and holidays.
  • Requires the ability to travel in the performance of job duties.
  • Requires ability to pass the IDHS background check.
  • Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
