The Illinois Department of Public Health (IDPH) is tasked with the oversight of communities within Illinois to achieve and maintain optimal health and safety.  With an annual budget of over $600 million in state and federal funds, 7 regional offices, 3 laboratories and over 1,100 employees, IDPH is equipped to fulfill its mission of protecting the health and wellness of the people of Illinois through the prevention, health promotion, regulation and control of disease and injury.

Position Overview

Under general supervision of the Regional Field Supervisor, conducts surveys, investigations and monitoring visits of long-term care facilities to determine compliance with State Licensure regulations and/or Federal Medicare/Medicaid certification regulations.  

Job Responsibilities

  1. Travels and conduct surveys of skilled and intermediate care nursing homes, shelter care facilities, veterans homes and related facilities to determine compliance with State Licensure regulations and Federal Medicare/Medicaid certification regulations.
  2. Prepares licensure and certification survey/investigation forms and reports of findings as required; Documents survey/investigation findings in accordance with the mandated Federal principles of documentation.
  3. Conducts on-site monitoring visits at non-compliant facilities pursuant to the monitoring requirements established by the Nursing Home Care Act.
  4. Participates in staff meetings and training sessions designed to improve survey performance.
  5. Performs other duties as required or assigned which are reasonable within the scope of the duties enumerated. 

Minimum Qualifications

  • Requires graduation from an approved nursing education program resulting in an Associate or a Diploma Degree in Nursing and three years of professional nursing experience or, a Bachelor’s Degree in Nursing and two years professional nursing experience or, a Master’s Degree in Nursing. 
  • Requires licensure as a Registered Nurse in the State of Illinois.

Preferred Qualifications

  • Prefers elementary knowledge in conducting surveys in skilled and intermediate care nursing home facilities and shelter care facilities.
  • Prefers 1 years of experience in conducting survey activities in nursing homes and related facilities to determine level of compliance with State Licensure regulations and Federal Medicare/Medicaid certification standards.
  • Prefers working knowledge and at minimum 1 year of experience of federal and state license survey reports and related compliance documents to document survey findings.
  • Elementary knowledge of the on-site process of monitoring visits to non-compliant homes as delineated by the Nursing Home Care Act.
  • Prefers 1 year of experience in the preparations of survey reports and the recommendations to supervisors regarding the status of the health and safety of residents in the monitored facility(ies).
  • Prefers excellent computer skills and work experience in Microsoft Word. 
  • Prefers experience working with laptop computer.
