Position Overview
The Illinois Department of Public Health is seeking a highly motivated, detailed oriented individual to serve as a Social Worker Surveyor 1. Travels to conduct routine health and safety surveys to determine the compliance with state and federal requirements. Interviews facility patients, staff, administrators, health care providers and other individuals. Investigates abuse, neglect, or exploitation complaints and discharge planning involving Hospice, Hospital and ESRD facility patients. As a member of the health facility survey team, explains and outlines deficiencies.

Essential Functions
- Performs surveys of all provider types under healthcare facilities and programs, including but not limited to Hospice, Home Health Agencies, Hospitals and End State Renal Disease Facilities (ESRD) to determine compliance with state licensure requirements, federal certification mandates, and Department directives, program policies and procedures.
- Investigates complaints relating to compliance with state and federal regulations involving health and safety of patients at healthcare facilities such as Home Health Agencies, Hospice, Hospital and ESRD facilities.
- As a member of the health facility survey team, explains and outlines deficiencies.
- Completes necessary surveyor training.
- Participates in staff meetings and assist with orientation of newly employed health facilities surveillance staff and other Department staff as directed.
- Performs other duties as required or assigned within the scope of the responsibilities enumerated above.
Minimum Qualifications
- Requires a master's degree in social work from a recognized college or university.

Preferred Qualifications
- One year experience with state and federal training concerning laws and regulations governing licensure, Medicare/Medicaid certification and inspections of health care facilities and to obtain federal Hom Health Agency, Hospice, Hospital and ESRD Qualification Training certification.
- One year of experience of the theories and assumptions underlying the social work field.
- One year experience preparing complex written and oral reports.
- One year experience in evaluating services provided similar to conducting a survey that includes the evaluation of the environment, records and care and service delivered by the Home Health Agency, Hospice, Hospital and ESRD care providers, identifying areas of non-compliance, problems and discrepancies, and determining possible resolution or action.
- One year experience in gathering, collating and classifying information about data, people or things and to interpret an extensive variety of technical materials in books, journals and manuals.
- One year experience maintaining effective working relationships with professional personnel, supervisors, peers, the general public and consumers.
- One year experience establishing, maintaining and improving cooperative working relationships with various stakeholders.
- One year experience critical thinking, judgment, discretion and maintain confidentiality during the conduct of the survey process.
- One year work experience in Microsoft Word, processing, and spreadsheet applications (i.e. Microsoft Word, Microsoft Outlook, Excel, PowerPoint, Smartsheet’s).

- Overtime is a condition of employment, and you may be requested or required/mandated to work overtime including scheduled, unscheduled, or last-minute overtime. This requires the ability to work evenings and weekends.
