Job Responsibilities

Performs nursing assistance and personal care services for residents with physical and/or mental impairment

  • Assists residents with activities of daily living-bathing, shaving, dressing, feeding, and elimination needs
  • Follows written and verbal instructions regarding resident ADL care, nutrition and activities
  • Instructs residents in the development of self-help skills to the extent they are capable
  • Ambulates, moves, and transports residents in beds, wheelchairs, stretchers, and lifts using the proper lifting techniques
  • Transports residents to and from outside appointments

  Performs the following treatment measures as directed by clinical staff

  • Takes and recording of weight and vital signs (blood pressure, temperature, pulse, and respiration, shampoos, and application of lotions and creams
  • Performs for or assists residents with range of motion exercises
  • Reports compliance with treatments to the nurse, such as: orthotic splints, wound dressings, skin wraps, oxygen therapy, and other related treatments

Continually inspects and checks residents in the assigned areas to ensure comfort and safety
  • Changes resident position as required per policy or ordered by physician
  • Observes and reports changes in resident’s behavior or physical condition/appearance to the clinical staff and documents observations on required documentation collection tools/devices
  • Completes assigned written documentation such as I & O Sheets, Bowel Sheets, and dietary intake sheets

Performs routine straightening and organizing of the residents’ units

  • Ensuring clean beds and resident bedside storage area
  • Cleaning patient care equipment
  • Maintaining general order of all nursing areas
  • Inventories residents’ personal clothing and belongings per policy and procedure
  • Empties trash containers from the bedroom and bathroom areas and transports dirty laundry to the soiled utility rooms
  • Serves as preceptor to orient new VNACs to the unit’s policies, procedures, routine, and tasks

Performs effective infection control practices utilizing universal precautions in resident care

  • Follows facility policies regarding antisepsis and disinfection procedures
  • Follows written and verbal instructions regarding resident ADL care, nutrition, and activities

Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above

Minimum Qualifications

  • Requires successful completion of an Illinois approved Basic Nursing Assistant Training Program consisting of a minimum of 120 hours of instruction which shall include at least 80 hours of theoretical instruction and 40 hours of clinical practice.
  • In compliance with Section 340.1376, of the Illinois Administrative Code, requires inclusion on the Illinois Department of Public Health Nurse Aide Registry.
  • This class is included as an Upward Mobility Program credential title.

Conditions of Employment

  • Requires the ability to pass a drug screen and physical
  • Requires the ability to pass a background check
  • Requires the ability to bend, stoop, twist, stand and walk for prolonged periods of time
  • Requires the ability to push, pull or lift up to 50 pounds unassisted
  • Requires the ability to remain in good standing on the IL Department of Public Health/Health Care Registry
  • Requires the ability to meet all agency vaccine/health-related policies and guidance
  • Requires a valid Illinois Driver’s License
  • Overtime is a condition of employment, and you may be required/mandated to work overtime including scheduled, unscheduled, or last-minute overtime
