Position Overview
The Illinois Department of Public Health is seeking a highly motivated, detailed orientated individual to review and process complaint and facility reported incident allegations of abuse, neglect and/or misappropriation of resident property. Participates and travels to conduct unlicensed investigations as directed. Provides clinical oversight and supervision to the Division’s Facility Incident and Monitor Program. Serves as a working supervisor and provides coverage for the Central Complaint Registry as needed or as directed.

Essential Functions
- Provides clinical oversight and supervision to the Division’s Facility Incident and Monitor Program.
- Serves as working supervisor.
- Processes facility incident reports to ensure compliance with program requirements.
- Travels to attend staff meetings, inter-agency meetings and workshops as a means of maintaining technical expertise in the field of long-term care.
- Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
- Requires knowledge, skill and mental development equivalent to completion of four (4) years college, preferably with courses in nursing.
- Requires prior experience equivalent to three (3) years of progressively responsible administrative experience in nursing.
- Requires licensure as a Registered Nurse in the State of Illinois.
Preferred Qualifications
- Extensive knowledge of federal and state rules and regulations concerning licensure and certification of long-term care facilities.
- Certified federal long term care surveyor, having passed Modules A and B of the Federal Surveyor Minimum Qualifications Test (SMQT).
- Three (3) years’ experience with nursing principles and practices.
- Three (3) years’ experience surveying in long term care facilities.
- Three (3) years’ experience analyzing and interpreting reports of survey findings.
- One (1) year experience exercising independent judgment as a working supervisor.
- One (1) year experience in hiring, performance improvement and performance evaluation.
- One (1) year experience developing and maintaining harmonious and productive working relationships with various administrative and medical personnel, local governmental agency personnel and recipients of health care programs.

- This position has been found to meet the requirements in section 4A-101 of the Illinois Governmental Ethics Act requiring the occupant/employee to file a Statement of Economic Interest. The Illinois Governmental Ethics Act (5 ILCS 420/4A et seq.) requires certain state officials and employees to file annual Statements of Economic Interest with the Office of the Secretary of State that will be made available for examination and copying by the public. All employees required to file a Statement of Economic Interest are also required to file the Supplemental Statement of Economic Interest with Executive Ethics Commission. (See Executive Order 15-09) Fines and penalties apply to untimely filings.
