Position Overview

Under general direction on an assigned shift and/or assigned home, supervises, coordinates, schedules, assigns and reviews the work of direct care personnel in the provision to non-professional services, programs and activities for individuals' served who are developmentally disabled; provides coverage to other units in the absence of their supervisor; assists the Unit Administrator and professional staff in the planning and implementation of habilitation programs; serves as a member of an Interdisciplinary Team assisting in the formulation and implementation of the Assessment/Habilitation Planning/Program and Service Implementation Process; coordinates and integrates shift routines/programs/activities.

Job Responsibilities

1.  Serves as a working supervisor.

 

2.  Monitors programs and activities for the personal care and treatment of the individuals' served.

 

3.  Serves as a member of the Interdisciplinary Team assisting in the formulation and implementation of the Assessment/Habilitation Planning/Program and Service Implementation Process.

 

4.  Provides instruction, training, and guidance to subordinate staff to verify proper program implementation and maintenance of daily activity/Active Treatment Schedules.

 

5.  Ensures that sufficient data is collected as indicated in the Individual Support Plans.

 

6.  Monitors designated living areas to verify equipment is in good repair/working order, areas are clean and free of hazardous conditions, painting/repair work has been completed.

 

7.  Completes and/or reviews forms, logs, and reports including injury reports, restraint forms, rights restriction forms, supply requisitions, etc.

 

8.  Provides coverage to other units in the absence of their supervisor.

 

9.  Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.

Minimum Qualifications

  1. Requires knowledge, skill, and mental development equivalent to an Associate of Arts Degree, with course work in the health and human services fields.
  2. Two years direct care experience in a mental health/rehabilitation residential care setting OR
  3. Requires two years individual care supervisory experience in a mental health/rehabilitation residential care setting.

 

Qualifying state employees in the Upward Mobility Program shall have the state requirements of this classification waived by successful completion of specific proficiency tests and training programs.

Knowledge, Skills, and Abilities

  • Requires thorough knowledge of individual treatment and care policies and procedures.
  • Requires working knowledge of the causes, nature, and treatment of intellectual disabilities, developmental disabilities and the methods and techniques employed in individual care, development and rehabilitation.
  • Requires working knowledge of the behaviors and problems of individuals with physical disabilities.
  • Requires general knowledge of the methods and procedures of supervision.
  • Requires working knowledge of household management, including cleaning, food service and supplies.
  • Requires ability to communicate effectively orally and in writing.
  • Requires the ability to assess operational needs and delegate staff.
  • Requires ability to supervise staff effectively.

Work Hours:  M-F 8:00 AM -4:30 PM 1 early day, 1 late day per week/6 week rotating weekend schedule, Weekends every 6 weeks
