Position Overview

The Division of Developmental Disabilities is seeking to hire a Social Service Program Planner to direct and guide the activities and services for transitioning people with intellectual disabilities to the community settings in Lake County.  This position assigns, directs, supervises the programs in the implementation of treatment and service plans including completion of tasks such as quality assurance checks, data collection, analysis, implementation of department of public health regulations, community integrated living arrangements and other applicable rules and regulations; provides direction and guidance with regards to behavioral problems; develops and implements policy that impacts services for the person; and verifies compliance with Waiver Rules designated be the Department of Human Services (DHS), Public Health and other regulatory agency policies, standards and regulations.

Essential Functions

  • Performs in-person complex technical tasks and develops complex program design plans for the coordination of service opportunities to individuals transitioning among serve types within the Developmental Disabilities system.
  • Develops, plans and implements in-person placement programs and policy.
  • Analyses SODC admissions process to determine needs in service delivery system.
  • Responds to request for technical assistance from individuals with developmental disabilities, family members/legal guardians and service providers.
  • Processes funding request for Community Integrated Living Arrangement (CILA) and /or Purchase-of-services (POS).
  • Travels to plan and conduct random and targeted on-site visit to serve providers in determining and reporting on the level of compliance in relations to Medicaid rules, other applicable rules, DHS principles and philosophy and best-practice criteria.
  • Performs other duties as required to assigned with are reasonably within the scope of the duties enumerated above. 

Minimum Qualifications

  • Requires knowledge, skill and mental development equivalent to the completion of four (4) years of college.
  • Requires one (1) year of professional experience in community organization, social service work or in program planning and development.

Specialized Skills

  • Of the one (1) year of required experience, requires one (1) year experience working with individuals with developmental disabilities, reviewing their Individual Support Plan’s and reporting their findings

Preferred Qualifications

  • One (1) year of experience working with individuals with developmental disabilities, reviewing their individual Support Plan’s and reporting their findings.
  • Two (2) years of professional experience working with program objectives of agencies developing social policy.
  • Two (2) years of professional experience working with structure, organization and methods of social welfare institutions.
  • Two (2) years of professional experience exercising extensive initiative and independent judgment in developing and formulating policies, standards and procedures for the service
  • delivery system.
  • Two (2) years of professional experience analyzing and evaluating programs and operations.
  • One (1) year of professional experience establishing effective working relationships with other staff, various groups and the public.
  • One (1) year of professional experience verifying program operations comply with all rules, regulations, policies and procedures

  • Requires the ability to meet criteria for a Qualified Intellectual Disabilities Professional (QIDP).
  • Requires the ability travel in the performance of job duties.
  • Requires ability to pass the IDHS background check.
  • Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
