Position Overview
The Illinois Department of Public Health is seeking a highly motivated, detailed orientated individual to serve as a field supervisor. This individual will oversee a multidisciplinary team that conducts surveys of long-term care facilities located in assigned Region, including state licensure surveys, federal certification surveys and complaint surveys. The Field Supervisor is responsible for assigning licensure, certification and complaint surveys, reviewing written survey documentation, testifying in court and performing monitoring functions.

Essential Functions
- Oversees long term care facility survey activities.
- Serves as working supervisor.
- Conducts state licensure, federal certificatoin, and complaint surveys of long-term care facilities.
- Develops long-term care facility survey schedules, reviews and processes survey packages and makes recommendations to Central Office.
- Completes necessary surveyor training.
- Represents the Division through participation in meetings with LTC industry representatives, advocacy groups, and other intereted parties as requeted.
- Performs other duties as required or assigned which are reasonably within the scope of the duties enumerate above.
Minimum Qualifications
- Requires knowledge, skill and mental development equivalent to completion of four years of college.
- Requires prior experience equivalent to three years of progressively responsible administrative experience in nursing.
- Requires licensure as a Registered Nurse in the State of Illinois.
Preferred Qualifications
- Three or more years of progressively responsible administrative experience in Medicare/Medicaid related programs.
- Three or more years extensive knowledge of state and federal regulations related to long term care facilities.
- Three or more years of supervisory experience.
- Two years of experience establishing, maintaining, and improving cooperative working relationships with various stakeholders.
- Two years of experience drafting, reviewing, and implementing policies and procedures.
- Two years of experience communicating business with various agencies.
Conditions of Employment
- Requires appropriate, valid driver’s license.
- Requires ability to travel in the performance of duties with overnight stays as appropriate.
- Overtime is a condition of employment, and you may be requested or required/mandated to work overtime including scheduled, unscheduled, or last-minute overtime. This requires the ability to work evenings and weekends.

- This position has been found to meet the requirements in section 4A-101 of the Illinois Governmental Ethics Act requiring the occupant/employee to file a Statement of Economic Interest. The Illinois Governmental Ethics Act (5 ILCS 420/4A et seq.) requires certain state officials and employees to file annual Statements of Economic Interest with the Office of the Secretary of State that will be made available for examination and copying by the public. All employees required to file a Statement of Economic Interest are also required to file the Supplemental Statement of Economic Interest with Executive Ethics Commission.
